Annual Chapter Compliance Paperwork

Starting in the summer season each year, all US Lacrosse chapters are required to submit a minimal amount of compliance paperwork. This includes the following:

1. Annual Chapter Board Report.
2. Financial Statements & Balance Sheet (now with the new Form 990 and e-Postcard requirements mandated by the IRS, the chapter has the choice to submit separate account information or the same information sent to be filed by the IRS.)
3. The newest version of your Chapter By-laws.
4. An updated, signed Chapter Agreement if the document was amended since last signed.
5. Signed Conflict of Interest Forms by all new members of the executive board.
6. Active D&O Insurance Policy through our administrator, Bollinger Inc. www.bollingerlax.com.
7. A complete Annual Participation Survey.

Once the required paperwork is received the chapter is considered compliant and can participate in any national events or programs in the following year.

If a chapter is not compliant by December 31 they will not be eligible to participate in some national events or programs in the following year. This includes the Chapter Rebate Program. If a chapter fails to meet the minimal compliance requirements they will not receive their 1st quarter rebate check or any after until the paperwork is received at national.

IRS Filing Requirements for small tax-exempt organizations in 2008

The Internal Revenue Service announced it began mailing educational letters to more than 650,000 small tax-exempt organizations that may be required to submit a new annual notice, Form 990-N, "Electronic Notice (e-Postcard) for Tax-Exempt Organizations Not Required to File Form 990 or 990-EZ."

The e-Postcard requires small organizations to provide a legal name and mailing address, any other names used, a Web address if one exists, the names and address of a principle officer and a statement confirming the organization’s annual gross receipts are normally $25,000 or less.

The IRS is developing a free reporting system for the e-Postcard and an application to make the information available to the public on www.irs.gov. Information about these systems will be announced as soon as it becomes available.
